As a new patient in our office, we do require the collection of personal, medical and dental information. We are committed to protecting the privacy of our patients’ personal information and utilizing all personal information in a responsible and professional manner.
For your convenience, you can download our patient form and complete it prior to your first visit. Then, bring the form with you when you come in for your appointment. If you choose to fill out the form at our office, please arrive 10-15 minutes before your appointment to allow enough time to get them completed.
